Schedule of Availability of Service:
            As scheduled by the Commission, twice a year
 
Who May Avail of the Service:
                        Qualified Filipino citizens who:

  •  Has a formal baccalaureate degree from a recognized institution of learning;
  • Has not been dishonorably discharged from military employment or dismissed for cause from any civilian position in the Government;
  • Must not have been convicted by final judgment of an offense or crime involving moral turpitude;
  • Must be at least one meter and sixty-two centimeters (1.62 m.) in height for male and one meter and fifty-seven centimeters (1.57 m.) for female; or if a member of indigenous peoples or tribal muslim must    have been issued a Certificate of confirm,ation or a grantee of height waiver; and
  • Must not be less than twenty-one (21) nor more than thirty (30) years of age:

  Requirements:

  • Transcript of Records or Diploma
  • Birth Certificate issued by the Philippine Statistics Authority
  • One (1) recent 1”x1” colored picture with name tag in white background
  • Height:   Male       – 1.62 cm (5’4”)

a.  Application letter under oath  of the claimant, , indicating briefly the identity of the deceased, date and
     the  cause of death (with 1x1 ID picture of the claimant
b.  Service Record (issued by DPRM)
c.  Certified copyt of the latest appointment
d.  Marriage Contract, issued by the Philippine statistics Authority (PSA) (2 original copies)
e.  Certificate of No Marriage Record (CENOMAR) of both parties, issued by the PSA (2 original copies)
f.   Birth Certificates of Children (if deceased PNP member was killed in action)
g.  Death Certificate of the deceased PNP member, issued by the PSA (2 original copies)
h.  Certificate of Clearance from Money and Property Accountability from latest unit
 i.  Latest payslip (authenticated by the Finance Service, NHQ)
 j.  Certificate of Last Payment (issued by the Finance Service, NHQ)
 k.  Detailed Investigation Report from the Chief of Police or immediate supervisor, indicating the
     circumstances or cause of death of the deceased PNP member (Spot Report/Progress Report)
l.   Complete clinical/hospitalization records from the hospital where deceased was confined
m.  Original copy of Official Receipts of Medical/Hospitalization Expenses, with Doctor’s Prescription
 n.   Proof of surviving legal heirs/Certificate of Beneficiaries (issued by the PRBS, NHQ)
 o.  Affidavit of two (2) disinterested persons (if the deceased is single)
 p.  Certification of Restorable Leave Credits for Absences Incurred by reason of the sickness suffered
      from or injury sustained as the cause of death of the PNP Member.
 q.  If claim is filed by a parent or by a brother or sister, proof must be submitted indicating that the deceased  died single without any surviving child.
 r.  Clearance  on administrative case from the following:

 All qualified children of PNP members so adjudicated, accompanied by surviving parent or guardian
